Each manuscript must be accompanied by a statement that it has not been published elsewhere and that it has not been submitted simultaneously for publication elsewhere.

The first page should contain the title of the article and information for each of the authors (titles, degrees and affiliations, mailing address, email address, phone and fax numbers). Indicate the person to whom editorial office correspondence and proofs should be sent. The second page should contain only the title of the article; this is for the purpose of blind peer-review. On the third page each article should be summarized in an abstract of not more than 150 words. Please list up to five key words or short phrases that indicate the major focus of the manuscript for indexing purposes. Avoid abbreviations, diagrams, and reference to the text in the abstract.
